Acceptance Packet/Visa Qualifying Documents

Once admitted, the IPE will express mail your acceptance packet and visa qualifying form (I-20 or IAP-66) to you or your home university. This form allows you to apply for an F-1 or J-1 visa at the nearest U.S. embassy or consulate. You will be given your UW student number at this time as well.

Apply for On-campus Housing

After you receive your student number, you may apply for on-campus housing. For more information, please refer to the Housing & Life in Seattle section of our website.

Measles Verification Form

All newly admitted students are required to provide proof of measles (rubeola) immunity. You will receive a Measles Verification Form with your acceptance packet. The form is also available on the Immunization webpage.

Students will not be allowed to register until this requirement has been satisfied.

Although you may mail in this form, we highly encourage you to fax a copy directly to Hall Health (Fax: 206.221.6628). This will help expedite the process.

Health Insurance / Insurance Waiver

The UW Accident and SIckness Insurance is required for international students and meets the minimum requirements of state law. For more detailed information go to http://depts.washington.edu/ovpsl/insurance.html.

As an exchange student, you are eligible to apply for an insurance waiver if you have comparable insurance that is valid in the US. Please note that you will still need to select UW Accident and Sickness Insurance when you first register. Once your waiver has been approved, then the ISO will remove the insurance charge from your account.
